Game Performance Assessment Index

Decsion Making Criteria:  Player attempts to make a pass when appropriate
                                        Player moves to open space to support a teammate
                                        Player attempts a lay-up when appropriate

Skill Execution Criteria:     Passing - ball reaches target
                                        Shooting - ball is shot with good form and is on target
                                        Recieving - ball is caught and player squares up

Support:                            Player supports the person with the ball by moving to open space

With this simple form you can determine what area the teams need to improve in most.  If there is a high # of possessions and a low # of shots, there is most likely a problem with getting open and maintaining possession of the ball.  If there is a high number of shots but a low # of goals, the students need to work on their shooting technique.  If this chart is used often, you can track the progress of the team throughout the unit.

This form provides some information about the adaptation of the learner to a game situation.  Again, you can tell the strengths and the weaknesses of the individual from the numbers in the different categories.  Involving students in the observation process will help them to see and understand the necessity and use of different skills.
